Requirements for Enrollment

*Tennessee Permanent Immunization Certificate

A complete medical exam for the first time enrollee

Birth Certificate

Social Security Card

Proof of Custody

Proof of Residence

KINDERGARTEN:

Kindergarten students have to be (5) five years old before September 30th.

ALL STUDENTS:

Proof of up to date immunization. By state law, the following vaccinations must be completed before a child is registered for school:

DPT and Polio (4 doses must be given on or after the fourth birthday. If not, then 5 doses are required.)

MMR (2 doses)

Hepatitis B (3 doses)

Varicella- Chicken Pox (1 dose for entrance into Kindergarten, 1st and 2nd grade or proof of illness.
